WHO WE ARE:
Sally Muich
Sally Muich, MSN, RN, is a Gerontological Nurse Practitioner and Director of The HouseCalls Program. Sally has over 23 years of nursing experience, including a variety of clinical experience in various settings. She received her masters degree in gerontological nursing from St. Louis University.
Dr. Kellogg
Deatrice Kellogg, MD is Medical Director of Assisted Care Living. Dr. Kellogg specializes in caring for the elderly and has over a decade of experience in both long term care and skilled rehabilitation settings in the Southern Illinois area.
Dr. Temporal
Michael Temporal, MD is the Medical Director of The HouseCalls Program. He is Associate Professor at St. Louis University Community and Family Medicine and a practicing physician with Southern Illinois Health Care Foundation. He is recognized in both 2008 and 2009 by St.Louis magazine as one of this area’s TOP DOCS
Donna Hofstetter
Donna Hofstetter is the HouseCalls Office Manager and assists in the scheduling of HouseCalls patient visits.

Many elderly adults find it either impossible or extremely difficult to get to their doctor’s office. HouseCalls by SIVNA provides the same services you would expect from a visit to your doctor’s office. Working directly with all area primary physicians and physicians specializing in elderly care, HouseCalls by SIVNA nurse practitioners and clinical nurse specialists come to you. All are nationally board certified at master’s degree levels.
WHO IS ELIGIBLE?
Persons who are home-bound.
Persons for whom office-based medical visits are a hardship.
