Do you or a loved one need assistance to remain independent in your home?  Services for Seniors of Southwestern Illinois Visiting Nurse Association may be able to help.  Services for Seniors is the Designated Care Coordination Unit for residents of Bond, Clinton, Madison, Monroe, Randolph, St. Clair and Washington Counties in Illinois.

A specially trained care coordinator will come to your home, free of charge, to conduct a thorough needs assessment and explain what community based resources are available to assist you or your loved one in living at home as independently as possible.  Our care coordinator will then work with you or your loved one to develop an individualized plan of care and make arrangements for needed services.

Local, state and/or Federal funding is provided for many community-based services. There are also services that a client can purchase with his/her own resources.
